I've used PSP9 for this tutorial, however you should be able to create this tag in all versions.
Supplies needed:
A tube of choice. I'm using one by Shawn McManus (http://www.shawnmcmanus.net) which is available from CDO (http://www.cdoestore.com).
Please do not use the same tube without the appropriate license.
Font of choice. I've used Thickhead.
Plugins:
Mura's Meister - Copies
Filter Factory C - Speed
Eye Candy 5: Glass
1. Open a new transparent image 600 x 250 pixels
2. Copy and paste your tube as a new layer and apply Mura's Meister Copies with the following settings:

3. Apply Filter Factory C - Speed with the following settings:

4. Apply Effects - Distortion Effects - Pixellate with the following settings:

5. Effects - Edge Effects - Enhance. Crop your tag back to 600 x 250 pixels.
6. Apply Eye Candy 5: Impact - Glass. Click on the Settings tab and select 'Clear'.
On the Basic tag, change the settings to match mine below:

7. Paste your tube as a new layer and add a drop shadow of choice.
8. Highlight your Glass layer. Selections - Select All. Selections - Float. Selections - Defloat.
Highlight your tube layer. Selections - Invert. Press delete on your keyboard.
Selections - Select None.
9. Add your name, copyright and license number (if necessary). Export your tag as a PNG.
To create your avatar:
10. Shift - D to duplicate your tag.
Crop it to 150 x 150 pixels.
11. Delete any unnecessary layers, making sure you left the background layer in place.
12. Apply Eye Candy 5: Impact - Glass with the same settings as previously.
13. Add your name or initial, plus any copyright and license numbers, as required.
Export this as a PNG as well.
And you're done :)
