I've used PSP9 for this tutorial, however you should be able to create this tag in all versions.

Supplies needed:
A tube of choice.  I’m using one by Psyco (http://psycojimi.deviantart.com), which you must gain a FTU license from My Tubed Art (http://www.mytubedart.com) to use. 
Please do not use unless you have the appropriate license.
Image of choice.  I’m using one I downloaded from www.sxc.hu
Fonts of choice.  You’ll need a bold font and a script font.  I’m using Pussycat and Affalite.

Plugins:
Fantastic Machine – Paint Engine
Graphics Plus – Pinwheel

1.  Open a new transparent image 600 x 250 pixels

2.  Set your foreground and background to two bright colours from your tube. 
Create a foreground-background gradient, angle on 45, repeats on 3.  Flood fill your canvas.

3.  Apply Graphics Plus – Pinwheel with the following settings:

4.  Copy and paste your image as a new layer.  Resize it and move it until you are happy with the image that is showing. 
Crop your image to 600 x 250 pixels.

5.  Apply Fantastic Machine Paint Engine with the following settings:

6.  Select your preset shapes tool, set to Eclipse.  Set your foreground to null, your background to a colour of choice.  It doesn’t matter what colour, as you will be deleting this layer later.  Draw out a circle, holding down the shift key to keep it even.  When you are happy with the circle, convert it to a raster layer.  Duplicate this layer.  Mirror the duplicate circle.  Make sure your two circles overlap slightly in the centre.  See my screenshot below:

7.  Merge your two circle layers.  Selections – Select All – Float – Defloat.  Highlight your image layer.  Selections – Invert.  Press the delete button.  Selections – Select None.

8.  Delete your circles layer.  Highlight your image layer.  Selections – Select All – Float – Defloat.  Selections – Modify – Expand by 10 pixels.  Add a new raster layer and floodfill the selected area with black.  Selections – Select None.  Move this layer under your image layer and merge them together.  Add a drop shadow of choice (I’ve used 2, 2, 70, 15).  Move it over to the right of your canvas.

9.  Paste your tube as a new layer and move it to the left of your canvas.  Add a drop shadow of choice (I’ve used 2, 2, 70, 15). 

10.  Add a new raster layer.  Selections – Select All.  Selections – Modify – Contract by 4.  Selections – Invert.  Floodfill with colour of choice (I’ve used black). 

11.  Add your saying (I’ve used Summer Days) in your bold font, using white or colour of choice.  Add a drop shadow (I’ve used 2, 2, 70, 7).  Change your blend mode to soft light.  Add your name using the script font, using black or other colour of choice.

12.  Add copyright and license number (if necessary).  Don’t forget to credit your background photo.
And you’re done :)

To create the avatar:
1.  Select the background layer of your tag and copy it.

2.  Paste as a new image and crop to 150 x 150 pixels.  You can do this with a custom crop:

3.  Add a new raster layer.  Selections – Select All.  Selections – Modify – Contract by 4.  Selections – Invert.  Floodfill with colour of choice (I’ve used black). 

4.  Add your name or initial using your script font.

Remember: No copyright is necessary on your avatar because you created the background using a gradient. 
And you’re done :)
