I've used PSP9 for this tutorial, however you should be able to create this tag in all versions. 
I am assuming you have some knowledge of how PSP works, including applying masks.

Supplies needed:
A tube of choice.  I'm using one by Lady Mishka, available from the TPP Vault.
Please do not use the image without the appropriate license.
Scrap kit of choice.  I'm using a PTU kit, Lick This by Toxic Desirez, also available from TPP.
Font of choice.  I've used Quilted Butterfly.
Mask of choice.  I've used Trese Masks0114 by Trese Tags & Tuts

Plugin: Eye Candy Impact - Gradient Glow (optional)

I've written this so you can twist your result to suit your tube and scrapkit choice - Hope you enjoy it.
I have resized pretty much all the elements to suit my tag - please do the same as necessary

1.  Open a new image 600x600 pixels.   

2.  Paste your tube onto your canvas, resizing if necessary.  Add a drop shadow of choice.

3.  Copy and paste a paper of choice and move it below your tube (I've used Paper 6). 
Apply mask of choice to this layer and merge group.

4.  Add any elements around your tube.  Add drop shadows as required.
I've used Beaded String, Flame, Star Border, Ribbon 1, Splatter and Frame 3 in my example.

5.  Crop your tag and resize if necessary.

6.  Add your name, using a font of choice.  I also added a gradient glow and dropshadow.

7.  Add the appropriate copyright and license number if necessary.  I've also credited the scrapkit maker.
And you're done :)
