I've used PSP9 for this tutorial, however you should be able to create this tag in all versions.

Supplies needed:
A tube or close up of your choice.  I'm using one by Karen Middleton which is available from CDO
Please do not use the same tube without the appropriate license.
Font of choice.  I've used Before The Rain

Plugins:
Mura's Meister - Copies
Filter Factory A - Ripper
AAA Frames - Foto Frame

1.  Open a new transparent image 600 x 250 pixels.  Paste your tube as a new layer.

2.  Apply Mura's Meister Copies with the following settings:



3.  Crop tag to 600 x 250 pixels.  Apply Adjust - Blur - Radial Blur with the following settings:



4.  Apply Effects - Distortion Effects - Pixellate with both block height and width to 10. 
Apply Effects - Edge Effects - Enhance.

5.  Duplicate the Pixellate layer.  Make sure you are on the duplicate layer. 
Apply Filter Factory A - Ripper with the following settings:



Change the blend mode to Overlay or one that suits your tag.
Merge visible. 

6.  Apply AAA Frames - Foto Frame with the following settings:



7.  Paste your tube as a new layer and move it to the left-hand side of your tag.  Apply a drop shadow of choice.

8.  Add a new raster layer and flood fill it white.
Selections - Select All.  Selections - Modify - Contract by 3.  Press delete.  Do not deselect yet

Selections - Modify - Expand by 2.  Selections - Invert.  Flood fill black.  Deselect. 

9.  Add your name and copyright and license number (if required).  I also added a drop shadow.
And you're done :)
