I've used PSP9 for this tutorial, however you should be able to create this tag in all versions. 

Supplies needed:
A tube of choice.  I'm using one by Wassim Jelassi, which is available from Artistic Dreams Imaging.
Please do not use the same tube without the appropriate license.
Scrap kit of choice.  I'm using Aloha by Lliella Designs, available from Sweet Shoppe Designs
Please use a kit of your choice.
Font of choice.  I've used Altast Greeting.

Plugins:
AAA Frames - Foto Frame
Eye Candy Impact - Gradient Glow (optional)

1.  Open a new image 600x250 pixels. 

As I have used a full size kit, I have resized all papers and elements quite significantly. 
Please resize your kit elements as required.


2.  Copy and paste paper of choice from your scrapkit.  I've used paper10 from my kit.

3.  Paste your tube and move it into position on the right side of your canvas. 
Add a drop shadow of choice.

4.  Add elements of your choice on the left hand side. 
I used a sun and flower, before placing some word art over the top.
Add any drop shadows you like.

5.  Crop your canvas to 600x250 pixels.  Hide your tube layer and merge visible. 
Unhide your tube.  If it's not already, make sure your tube layer is at the top of the layer palette.

6.  Highlight your merged layer and apply AAA Frames - Foto Frame with the following settings:



7.  Add a new layer above your tube and flood fill it white.

8.  Selections - Select All.  Selections - Modify - Contract by 1.  Press delete.  Deselect.

9.  Add your name, copyright and license number (if required).
I added a Gradient Glow (see my settings below) and drop shadow to my name.



And you're done :)
